URGENT!! Please help - Given Name(s) in passport need to be bifurcated?

Hi,

In my passport, currently Given Name is "Revathy Padmanabhan". Surname field is blank. I have to change it to - Given Name - Revathy; Surname - Padmanabhan: just bifurcation of my name, not really a name change. Can you please let me know if there is a procedure to put an observaton stamp in my passport indicating my first and last names. If not, issuing of a new passport is the only way?

Also to make matters complicated, my current address is different from the address given in the passport. Will it create problems? If I get a new passport issued for bifurcation of my name, can I also ask for a change of address simultaneously; or does it have to be routed as a different request.

Please reply asap. Thanks in advance.

go to your passport office  they will re write your name   correctly on the last page   they will stamp you passport   amended
