  1. The standard is generally that most insurance companies will allow "medically necessary" ultrasounds during pregnancy.  In other words, they won't allow you an ultrasound just to determine baby's gender, etc.  My OB/GYN routinely did an ultrasound at about 7 weeks and about 20 weeks.  (Other doctors may have different recommendations - that's just what my doctor did for both of my pregnancies.  And with my son, I had to have an "extra" ultrasound at 26 weeks b/c they wanted to monitor something unusual they saw on my 20 week ultrasound.)

As long as your doctor can justify a reason for doing the ultrasound, you're generally going to be okay.

  2. Normal pregnancy:  One at around twenty weeks. However, if you are unsure of conception date, then usually the first is at a guessed eight weeks, second at twenty.

High risk: Depends on what is causing it to be high risk.  If the doctor can justify it, insurance usually pays.  My sister-in-law had to go in either weekly or bi-weekly due to an abrupt placenta.  My wife got a third one on our first child since she was so dehydrated near the end.
