I think it might be possible, assuming that WWE wants to recycle one of last year's storyline and thus assuming Triple H retains the title after the Championship scramble.

The storyline that might be recycled would be that which took place in No Mercy 2007 in which Triple H had 3 title matches in total that night. The first match was when he won the title from Randy Orton, the second one when he retained against Umaga, and the third one when he lost the title again to Randy Orton.

So there's a possibility that after Triple H has successfully retained his title, an authority figure, which I think would be Vickie Guerrero would suddenly appear. Since she's so frightened of The Undertaker who claimed that he would come for her soul and threatened her with all his mind games, she would declare at that moment that she could not take it anymore and she might want to give Undertaker a title match at that moment (which is for WWE title since the title is Smackdown's main title) so that Vickie would be forgiven and thus Undertaker won't disturb her anymore.

The match would begin once Unforgiven is supposed to be over (after the last match of the day, which is so far the ECW Championship scramble). Triple H would defend his title in a match with Undertaker, and Triple H loses this match, making The Undertaker the new WWE Champion.

Just my own opinion, since many people have asked when WWE is going to get rid of the current spinner belt. Firstly, I don't think Undertaker would be suitable to hold this spinner belt as it would look weird on this gimmick. (assuming that Undertaker won't change gimmick to bad-*** again, which is unlikely anyways) Thus, if Undertaker does win the title in Unforgiven, it might lead to a new belt design for the WWE championship, maybe a black colored belt with death theme? But if that's so, they must give Undertaker a lengthy reign otherwise when someone wins it, then this "dark belt" would look weird on the new winner. We shall see though.
