Unemployment Form - wages earned ?

I'm filling CA Unemployment Form, and one of the question requires to provide the following information for each employer I worked for in the past 18 months:

...

33b. Employer name

33c. Beginning and ending dates of employment

33d. Wages earned

33e. How you were paid (specify hourly, weekly, monthly, annually, commission, or piece rate)

So I got confused on the Wages earned question. If I list that I worked at that company for 3 years, should I sum all the wages earned during those 3 years?

  1. No just the wages that you earned in the 12 month period ending last week.  so if you list job (a) typist and you earned 12,000.00 /14 months ago you have to divide by 14 to get the total for the 12 month period....or about 10,000.00 dollars .
