Uneven pupil dilation?

Last night my vision started getting a little blurry, and my eyes seemed a bit irritated, so I looked to see if they were red at all. My right pupil was quite dilated, although it did respond to variations in light. The on-call doctor urged me to go to the ER for a CT scan, which I did. They did numerous tests along with the scan, and everything came back normal. The ER doctor thought I may have a neurological problem; I've already been trying to get an appt. for some other issues. Otherwise I have a follow-up with my family doctor tomorrow.

What else could cause uneven pupils? I have had no injuries, falls, etc., am not a smoker, not diabetic, no high BP.

  1. Adie's tonic pupil comes to mind, but that generally stays fixed and doesn't react to light.  You could have an inflammation of the eye which could cause the uneven appearance.  Definitely, you need to see an ophthalmologist or neurologist....a neuro-ophthalmologist would be best i there's one in your area.


  2. Having one pupil larger than the other is not normal. It could be Horner’s

    syndrome which is a paralysis of the oculosympathetic nerve. The problem is however not always benign. Uneven pupils or anisocoria, may be of no pathological significance, but

    pronounced difference in the size of your pupils can be symptomatic of an organic lesion

    such as an Aortic aneurysm, tumor, enlarged thyroid, glaucoma and intracranial pressure.

    If the abnormal pupil is the smaller of the two that is usually caused by hyperemia ( increased blood flow to the eye)

    of the iris or iritis. Your eye would appear bloodshot.

    Have someone examine your eyes. I suggest that they look at your pupils. If, when they look at your pupils in ambient

    light ( no flashlight or something similar) and they appear to you to be distinctly

    uneven then I would call an ophthalmologist. Make sure you state to the

    receptionist that your pupils appear to be uneven. If you can’t see the specialist

    then I suggest that you see your family doctor as soon as possible to rule out any

    complications.

  3. Man, that sounds like a scary thing to happen. I was thinking a bleed inside your head, but you'd expect the CT to have picked up something like that. Maybe it's some kind of eye disease. I hope you get it sorted.
