Application Requirements

You must apply for a passport in person but do not require an appointment.  Please bring the following with you to the Consulate:

Your most recent United States passport;

One of your parents (or a legal guardian) and his/her valid U.S. or foreign passport;

Any foreign passport that you may currently hold, have held, or been included in;

Name change documentation: If you have changed your name since your most recent passport was issued, you will be required to furnish name change documentation. Please follow this link for additional application requirements;

need to bring the passport, the child with one custodial parent (assuming both parents have custody of the child and both have the same last name - if not, check the section in the state departments web on special requirements for children), two new  

regulation passport photos and the parents drivers license.
