Using the NHS under a spouse visa?

I am currently living in the US, married to a UK citizen. I have applied for and received a spouse visa entry clearance valid for 2 years. Can I receive an NHS number when I arrive in the UK, or do I have to have private healthcare? It seems that I might have to wait 2 years once I apply for Indefinite Leave to Remain. Can anyone help out? We leave on Thursday! Thanks Jay
