Esquires Coffee Houses - there's one in Bby. on Kingsway, and a few more scattered around the city (One at 41st and West Blvd. in Kerrisdale). They use real chocolate (cacao) and you can get it with whole or low fat milk.

Cheesecake Etc. on Granville has the most (and I mean MOST) delicious hot chocolate ever. So rich and topped with whipped cream, it is perfection!
