Vanguard 401 (k) termination form question?

I'm filling out my 401k termination from Vanguard, I signed a couple waivers, but there's a section that says "To be completed by a notary". Can I send it back blank?

  1. Unfortunately no.  You must have a notary witness your signature and sign and possibly stamp the form.  Go to your local bank, and they will notarize it for you.  

    If you just send it back blank, they will only send it to you again and further delay your getting your money.


  2. Sure, but they won't do anything with it!

    You have to sign in front of a notary to have it notarized!

    Try your bank, someone there will do it...
