Velbon 5000

I am trying to figure out how to use my Velbon 5000 tripod. I never use tripods and do not use my camera that often, but have gotten it out so I can use it at Christmas. I feel like I may be missing a piece or something, because I can't see how to connect the camera to the tripod. Can you give me specific instructions? Thanks.
