Vessel Statement from office of assessor. Do i have to have this filled out by a professional?

I just recieved the tax form for my new boat and I'm not sure if I can fill this out myself or I need to have it assessed by a professional.

It's for San Bernadino County, CA

The boat is a 25' Crusier with a Merc 350

I am not sure how it works in California but when I had to do something like that in NC I had to present my bill of sale for the boat showing when we bought it and how much we paid for it.  Then a certain percentage was allowed for depreciation for each year that we owned the boat.  Can you go in to the tax office and ask them, or is there a phone number you can call?  Most likely it is a simple thing, and someone there can quickly tell you what you need to do.  Best of luck with getting it sorted.
