Visa Gift Cards: Buying at store and buying at the bank?

I know you can buy them at stores in increments such as 50, 100, 200. But I hear that you can buy them at the bank and have any amount loaded onto them. I was just wondering When you go into to the bank where do you find them and who do you ask for the amount you want. Also, when you buy them at the store they charge an activation fee, do they also charge this activation fee at the bank?
