  1. Yes you do. You can look up the nearest US Embassy in this link.**

  2. Yes, you will need a visa if you are not a Canadian citizen.

  3. ALL YOU NEED IS YOUR GREEN CARD AND DRIVERS LICENSE UNLESS YOU'RE FLYING THEN YOU MAY NEED YOUR PASSPORT. SOUNDS SCARY, BUT IT WASN'T FOR ME FOUR MONTHS AGO WHEN I WAS DRIVING FROM IOWA TO NEW YORK AND FOUND MYSELF ON THE US CANADIAN BORDER. WHEN I ASKED THE CANADIAN CUSTOMS WHERE I WAS IT TURNED OUT I WAS IN CANADA. SCARED FOR FOUR HOURS DRIVING IN ONTARIO HOW WILL I GO THROUGH US CUSTOMS? ALL THEY WANTED WAS MY GREEN CARD AND DRIVERS LICENSE AND BACK INTO THE US I WAS.

  4. The PR card is not a valid travel document. It cannot be used to enter another country. It can be used as a valid identity document to foreign authorities but it will not allow entry to a country

so therefore contact the US consulat in Canada for visa application

go to this page to find the nearest consulat in Canada

left side of the page , all locations of Us consulats  are listed
