Voe form for homeschooled kids?

ok i am home school i dont have a privte teacher i just buy my books online so who is going to fill out my voe form for a permit at diving school

Your parent is your teacher, so they would fill out the form.  Not all states require homeschools to be listed with anyone, much less the district; your parents are considered to be your principal, teachers, and guidance counselor.

A home School is listed with a district.  Either your local school district or a local church based school. Either the representative there, or your Parent.  (In a home school, your parents are considered to be your Teachers.)

Even if you work independently, your parent is still considered to be your teacher and the one fully responsible for your education, so your parent would sign it.
