  1. Send it back to Sony as this is a problem usually related to the firmware. This has happened to 2 of my ps3 hardrives. I finaly switched my ps3 in for an xbox 360 and have never regretted it.  


  2. It sounds like your hard drive is corrupted. You will either have to send the console in to Sony for repair (if you're under warranty it will be free), or just buy a new hard drive.

    You can use pretty much any 2.5 inch 5400rpm SATA laptop hard drive as a replacement. A cheap place to get said hard drive would be http://www.newegg.com . You can use this opportunity to upgrade to a larger size as well.

    If you don't want to upgrade (and pay for a new HDD) then just call Sony up. It will take a few weeks to get the PS3 replaced/repaired.
