Keep them in as long as you're still bleeding. Change them every 30 to 45 minutes, 45 minutes ideally. This is because you don't want to be disturbing the blot clotting process. The more you take gauze in and out of your mouth, the harder it will be for the blood to settle and clot. The longest you should still be bleeding would be 24 hours after the surgery. If you're not bleeding too much by the time you go to bed, you don't need to have the gauze in your mouth. A little blood may look like a lot when it's mixed with your saliva.
