  1. There might be a minimum height for some airlines, but it is based on your ability to reach the controls. I have known some very short people to get licenses. One was 4'10" and SHE had a "waiver of demonstrated ability" from the FAA that allowed her to fly with special platform shoes. She is now an airline pilot.

I don't think there is a minimum height requirement.  Anyone can get a commercial pilot's license, if you can pass the tests.  To actually fly as a commercial pilot, you would have to get hired by someone, and I don't know if there are any height restrictions.  You probably have to be tall enough to reach the yoke and pedals, but I've seen some pretty small people in the profession.  I'm not sure what -4 means in glasses.   Try the FAA website for information on medical qualifications, or just call an AME, or aviation medical examiner.  How much it will cost depends on a lot of factors.  You could find ways of only spending 20k, or it may take up to 100k at one of the aviation colleges.
