North Korea is easily the worst.  There are a few in African that come close but not for the same reasons.

Right now, Zimbabwe would be a good candidate.  Afghanistan and North Korea are also in the running.

Don't forget Mynamar (Burma).  Their government wouldn't help anyone after the worst natural disaster ever to hit that country.

Zimbabwe or the Dafur region of the Sudan. North Korea is pretty bad for many of the people living there.
