  1. Generally the first song is the couple's dance which happens shortly after the bride and groom are announced.  However, there's no set rule.   In my wedding we chose to eat first and played instrumental music and soft rock-type music.  Once the meal was over, we had our fist dance and then it was a mix of pop, classic rock and a couple of line dance songs.  We let the DJ set the order of everything (other than eating first--we had some people form a Dutch Reformed church at our reception, and they didn't believe in dancing.  This allowed them to attend the wedding and part of the reception.  They left after we had our first dance.)

  2. An example -

    5 p.m. Cocktails

6 p.m. Dinner, with program and toasts - you choose who you want to do the toasts at the program

    about 8 p.m. Cake cutting

about 8:30 p.m. First dance, parents dances, wedding party dance, family dance, open dance for all -- whichever you pick

about 10:30 Bouquet and garter toss, just before late lunch

Then dance resumes for however long you have the hall and liquor license.
