Were can I get a "RJ11 Modular Dual Port Surface Mount Jack " or something around that type of port?

my brother accidentally ripped a cord out of the back of a phone jack that was helping keep the internet and the 2 phones hooked up. So I went looking for what its called and there ya have it. But I don't know where to go and find 1 of these jacks at.
