Whacky birthday party (23) ideas!?

Question

Girl turning 23.

Going to be outside (roof terrace) in spring in spain (10-20ºC).

Love to dress up, decorate (low budget) and cook, if it can be prepared in advanced.

Group of about 20 and alcohol WILL be involved.

Looking for interesting / whacky / different and fun birthday theme ideas. If it's a really good idea I'll invite you to the party!

Its not so much a theme, but the addition of the game "twister" seems to wacky up any party.

One that is common in my area is having a sign that says "you honk, we drink". Its the drinking game that passing motorist can take part in.

Funny hats as a dress code requirement.
