Cut or chop a "V" into the crack. Use a concrete adhesive liquid and coat the surfaces of the "V". Mix cement mixture and fill.

You can buy concrete crack filler at any hardware store.  Just pick some up and follow the directions.

If you just have some cracks, but the driveway is still solid, all you need to do is fill the cracks with caulk to seal and keep them from spreading. If the cracked area is also loose, remove the section that is cracked and re-pour new concrete.
