"Natural" presumably means the caffeine was extracted from caffeine-containing plants. There's no fundamental difference between that and synthetic caffeine, although natural caffeine, depending on how thoroughly refined it is,  might also contain other related compounds. This is often an important difference for vitamins and nutrients, but probably not significant for a stimulant drug such as caffeine.
