  1. None.  They are, in mapping jargon, "feature types" that are arbitrarily assigned by municipalities or the developers of residential subdivisions or commercial properties.  In my line of work (and this relates to my line of work) I've seen  "boulevards" too narrow for two cars to pass each other.  What's infuriating are the developers who think it's cute to use the same names in a subdivision and give each a different "feature type".  Holly Lane, Holly Drive, Holly Court, etc.

  3. A boulevard is determined by a minimum width from side to side. Generally a boulevard has a walk way down the center.
