What's the fastest and most efficient way to obtain my birth certificate and social security card?

I'm 17 years old and live in New Jersey, my parents lost both documents...is there any way I can regain these documents?

  1. Because you are a minor, your parents will likely have to get these for you, as it requires ID to get them.  If you have a driver's license, you can use that as ID.  However, you cannot even get a driver's license without both of these documents.


  2. Go online to the New Jersey Vital Statistics office as well as the Social Security Administrations website.  Each will give you the specifics of reordering these records. Don't be surprised if you will need your birth certifiate to get the SS card replaced. (Will need proof you are you).  Since you are 17 your mother can also request them.

  3. If you were born in New Jersey get your birth certificate at:

    New Jersey State Department of Health and Senior Services

    Bureau of Vital Statistics

    P.O. Box 370

    Trenton, NJ 08625-0307

    For your SS card go to your local Social Security office and ask for it.  They will mail it to you.
