  1. There is a very simple form that the parents and child fill out. It costs anywhere from $30 to $250. RARELY, there will be a hearing. Otherwise, the judge will review it, and either approve or deny the name change. I've never seen one denied.


  2. You go to courthouse, get a form, pay the fee, and then the name is legally changed.

    It's same for last names.

    HOWEVER, if the birth certificate is incorrect, you do a different form, pay a smaller fee, and then your BC is changed.

    IE, your name is Sierra, but it's supposed to be spelled Ciara.
