  1. depends where it is, in islington it's a bistro & as pointed out £7 more expensive, in run down area's a cafe & in most other places a restaurant.

  5. restaurant has standard items on menu. if the same store makes it a bistro they have specialty items they are selling. Even McDonald's now has bistros. They sell fancier food.

The Russian royalty before the Communits revolutions vacationed in France.  Apparently, they liked their wine, women, and song to be brought very quickly, so they used to shout Bistrovia! which meant hurry up. The smaller restaurants which complied with quicker service became known as bistros. Today, there is not a lot of difference. Since a small fancy restaurant and a large bistro would be virtually the same.
