  1. It should have a date, state who you are and relationship of the minor childred, names, ages, who they will be travel with and relationship to minor children. where they are traveling to, the dates of travel, and return date.


  2. first off they should list childrens names and ages, where they are leaving from and going to and the projected time of stay

    in that country..contact name and # in said country..for emergency purposes,,,must be witnessed & signed by both the ex. that is allowing the children to go and by the ex that is taking them......

  3. It should just say that the other parent gives (name of parent) permisson to take (names of children) to (country). That is the main thing in the letter. If you have further questions call an airline company and ask or you can call the place where the letter is notarized. My parents do this often when we travel to mexico.

  4. You need to write down all the details of the trip, where they are going, how long, etc. and that you are aware and approve, then take the letter to a Notary Public and sign it in front of that person.  They stamp it with a raised seal.  It is legal proof that you wrote the letter.  Look in the yellow pages for a Notary Public.
