  1. Atropine, as much as necessary to counter the symptoms.


  2. Matrix is right. So many people have taken ACLS courses, in which the "maximum" dose of 3 mg is drilled into them, that it's often a shock to them when they see their first case of serious organophosphate poisoning, where massive doses of atropine are the rule.

    I wouldn't worry too much about 2-PAM, despite the meta-analyses: the mixture of classic organophosphates and carbamates in the studies makes for a confounding variable that can't be teased out easily.

  3. Atropine can be used as an antidote in conjunction with pralidoxime, though the use of "-oximes" has been found to be of no benefit, or possibly harmful, in at least two meta-analyses. Atropine blocks the parasympathetic nervous system, both vagal effects on the heart.
