  1. passport and visa- MINIMUM.  Contact the US State Department.

  2. You will need a current passport and a valid US visa in order to be allowed to board a plane to the US.  At the US port of entry, your documents will be examined and you will be asked a few questions to verify your non-immigrant intentions.

In order to receive the visa, you must apply at the nearest US foreign mission.  First you pay a non refundable application fee, then schedule an interview and fill out the DS-156 (and other forms, if warranted) on-line, print them and bring them with you to the interview.  When you arrive at the interview, you will need also need your photographs, and any additional documentation that supports your intentions.  See the US Embassy New Delhi website below for detailed information:

  3. The only mandatory documents are a current passport, valid for at least six months and a completely filled out visa application. Plus, you do have to pay the fee, of course. Visitors are required to prove to the satisfaction of the interviewing officer that they have strong ties to their country and will return after a short trip to the US. people bring in all sorts of documents to help them prove that, but none are actually required by law.
