What are some of the most common drugs you see errors with for renal or hepatic dose adjustments?

This is a tough question.  Since people with kidney and liver problems generally need several of the most common medications, it would be the most common medications that require adjustment, probably antibiotics, anticoagulants (blood thinners), and blood pressure medications.

The real difficulty, though, is when somebody has rapidly changing renal function, such as in severe dehydration or severe illness.
