  1. Kill the police

  2. Here is a good one: You have the last bottle of Retin-A in Bergdorf-Goodman on a Tuesday afternoon at 3PM, after the Luncheon.

You wanna see vicious? The Octagon got nothin' on this.

  3. Going after the last cigarrette and the last beer at the same time. Also, dueling with pistols could be close.

Really, though.  Games of death are really not games.  A game is something that, at some time, involves a degree of sportsmanship and honor. Even in dueling and chicken, neither of the people go in without at least thinking they have something in them that will help them win.

Even a lot of people who want to die, normally don't "invite" someone else to "come play with them" because, by then, it really doesn't matter whether they have company or not.  And, if one or both really want to die (believe me..I've been there), then it should not be a group thing. Especially since, if the two of you really want to die, then how does the one who doesn't "win" feel?  I know that I would feel like c**p if I already were "in that place", and couldn't even win at russian roulette!.

  4. Among mainstream sports/games, baseball typically has the highest fatality rate.  3 or 4 people a year usually perish in this edgy bloodsport

  5. MAFIA

  6. I remember a scene in "Gladiator" where they annoy a cobra in a box, let it out, then have to grab kernels of corn from around the snake.  I don't know if that was ever really a game, but it would count.

There was an earlier version of russian roulette using spring loaded daggers.  A person would make a wager, then pick from an assortment of daggers, all but one of which had a spring loaded blade.  The game master would then thrust the blade into the player's chest.  If he picked a spring loaded blade, nothing happened, he got paid.  If the blade were fixed, well, he lost the wager and his life.

There's the great scene from "The Princess Bride" with the iocaine powder.  Poison a cup, let your opponent pick, and you both drink.  One of you winds up dead, unless you've built up an immunity to iocaine powder.

The TV show "Alien Nation" had a cool version of russian roulette using a pump with multiple nozzles and saltwater, which was acid to them.

The movie "Intacto" is based on gambler's obsessed with random luck.  Some of their games, like being blindfolded and walking into traffic, would fit.

I know I'm just relating movie scenes, but I don't have much direct experience with death games.
