What are some reasons businesses choose not have Flex Spending Medical programs?

Question

If a business has other section 125 programs like offering health insurance, why may they not offer the Flex Spending so that employees can use pre-tax dollars for things like glasses and their copayments?

Can't think of any.  The only reason I initially thought they wouldn't do it, was because of the admin costs.  But if they have flex spending for dependent care, etc, then they already have the admin costs - I can't see why they wouldn't do medical.
