A map in a GIS could have been digitized at too small of a scale, thus lacking detail of some features.  Also, the map data may not be projected correctly.  There could also be problems with the registration of aerial photos, which could also lead to digitizing errors if anything is digitized from those photos.  Of course, one could also have typos or mistakes in the attribute data.
