What are the 5 types of transitions?

I do no know how you want ot group them. Here are a few:

- from room to room (class to calss)

- from class to break, lunch, assembly and back

- from home to preschool

- from preschool to elementary

- from elementary to middle school

- from middle school to high school

- from high school to a transition program, jobs, housing changes, college or other adult life details

That is why IDEIA mandates that transition plans be in place for all students with disabilities age 16 and older.  That is also why some students are in early intervention programs or have various types of behavior support plans in place.
