  2. i cant tell u in technical language but basically its a plan for the care and treatment of someone (in my case my mother who has dementia) it gives a case history, details of all the next of kin, doctors treating her etc, her likes and dislikes and objectives of her care. hope this helps a bit. its given to the social worker, the family and any carers so they have all the information they need

  3. Basically..A care plan addresses a problem with a solution.

    What needs to be done and by whom..

    A care plan will show the problem...goal..and interventions needed to acheive the goal...and who will do what interventions.

    Careplans more importantly should have a time limit...i.e. "Within 6wks..such and such will do such ans such to achieve such and such.

    So it can be tailored to any problem
