What are the different CST / ST forms used for transportation of goods (inter / intra states) in India?

what are the different CST / ST forms that are required for inbound and outbound transportation of goods in different STATES IN INDIA? WHO ISSUES THESE FORMS AND WHAT ARE THE CONDITIONS FOR IT ?

There are two CST in Mumbai they are one the CST railway station and another one is the CST airport.The govt issues the forms by just paying what they ask.

At the time of transport of the goods, it must be accompanied by sales invoice bearing the VAT No.of the sellers and full details of the goods including VAT, if any. In the case interstate transaction, for claiming concessional rate under the CST Act, C Form should be obtained from the buyer .The C Form is issued to the dealer of the buying state by the concerned Commercial Tax Officer.
