  1. The odds of this occuring are WAY less than the first answer.  (1 in a million?  I don't think so.)

I've seen it happen myself a few times and there's no way I've played anywhere near a million hands.  If you're asking because you saw it happen, then you can probably say the same thing.

I'm pretty good with odds and probabilities, (which is why I looked at your question) but the exact probability for your question is kind of tricky to determine.

You need to determine the odds of someone, for example, having a pair in their hand, and then the odds of another person having a pair in their hand, and then those same cards appearing on the flop.  Of course, a person doesn't have to have a pair in their hand to make four-of-a-kind... they can have a single card and three-of-a-kind can appear in the community cards.

  2. Check fulltilt.com's information about the Bad Beat Bonus. It should be about the same or slightly less than their posted odds.

  3. Well I play Texas hold em all the time. It depends on what you mean. Did both players have pocket pairs. Did one flop quads and the other person went quads. Either way it is very rare and you will probably see it happen once in a million hands.
