What are the processes of NAME CHANGE in OKLAHOMA?

Ok, so, I have gone to the Law Library in my county courthouse and got the forms that are supposedly helpful to me in order to change my two daughters surname. However, it [process] is very confusing, so, what are the actual processes to get it done, accurately? And no one at the Law Library are permitted to help me figure out what an I suppose to fill out in the blanks, I mean, I know what I want to change my daughter's last names to, but the way they worded it, I am just confuse. And, I want to avoid going to any attorney , if possible. Thanks!

    Chapter 33 §1631. Right to Petition for Change of Name.--This section basically says if you are a natural-born citizen who has been living  within the state or on a military base for at least 30 days, or an actual resident of the county or such military reservation situated in said county, or county in which the military reservation is situated, for more than thirty (30) days the following month before the filing of the action, may petition for a change of name in a civil action in the district court. If the person is a minor, the action may be brought by guardian or next friend as in other actions.

    1632. Required Context of Petition.

    In your petition you must provide: (a)  The name and address of the petitioner; (b) The facts as to domicile and residence (how long you have lived at such-and-such address, in other words); (c) The date and place of birth; (d) The birth certificate number, and place where the birth is registered, if registered; (e) The name desired by petitioner; (f) a CLEAR explanation of why you want the name changed (example: my ex-husband is a child molester);  (g) A positive statement that the change is not sought for any illegal or fraudulent purpose, or to delay or hinder creditors (you can't say you want the name change to escape paying income tax, for example, or for non-payment of creditors)

    §1633. Notice - Protest - Hearing Date - Continuance:

    You have to PUBLISH the fact you are changing your or your daughter's last name in a NEWSPAPER (usually in their classified section about court cases, legal notices, etc) 10 days BEFORE the hearing if said newspaper is  "authorized by law to publish legal notices printed in the county where the petition is filed if there be any printed in such county, and if there be none, then in some such newspaper printed in this state of general circulation in that county".  For instance, if your hometown does not have a newspaper with this sort of ad section, then you use one at a county level, or even a state level (a newspaper published state-wide). The notice shall contain the style and number of the case, the time, date and place where the same is to be heard (such as "I, Jane Doe, plan to change my surname as of June 4 at 10 a.m in Jones County Courthouse, case number 24..."  You must realize that someone may challenge this name change before the actual hearing, and also that the judge may "continue" (or reschedule) the case to a later date, sometimes done to investigate the truth of the petition.

    Chapter 33 §1634. Proof of Petition by Sworn Evidence: This section basically says if the proof (sustained by sworn evidence), of your petition is true, the name change will be granted. But, if the judge or court finds it all to be false (sought for an illegal or fraudulent purpose,),it won't be granted.

    Chapter 33 §1635. Judgment - Authenticated Copy:

    The judgment shall recite generally the material facts and the change granted, or if denied, the reasons for the denial. A certified or authenticated copy of such judgment may be filed in any office, where proper to do so, and shall be regarded as a judgment in a civil action. (If your daughter's name change is granted, in other words, you may notify her school, etc. about the change)

    Chapter 33 §1636. Fraudulent Use of Judgment a Misdemeanor--

    "Any person who obtains a judgment under this act, willfully intending to use the same for any illegal or fraudulent purpose, or who thereafter willfully and intentionally uses such judgment, or a copy thereof, for any illegal or fraudulent purpose, shall be deemed guilty of a misdemeanor."

      (If you change your daughter's name for fraudulent purposes , you can charged with a minor crime, and possibly serve jail time or pay a big fine).

    Chapter 33 §1637. Changes of Name to be Effected by this Act - Exceptions.

    After the effective date of this act, no natural person in this state may change his or her name except as provided in

    Sections 1631 through 1635 of this title and Sections 1 and 3 of this act, other than by marriage or decree of divorce or by adoption.

         (After the date of this name change, the only way your daughter can change it again is by getting married, divorced, or is adopted by someone else).
