What are the requirements for exporting my dog from the philippines to UK?

I am from the Philippines and in few days I will be leaving for UK. I am planning to bring my pet dog there. He is a 5-month old Shih Tzu. What are the requirements so that I can bring my pet? What are the restrictions of importing/exporting a pet to UK? Does my pet need to comply all his vaccinations? If so, his vaccinations are complete including rabies.
