What are the rules in setting a table?

I believe you are supposed to place silverware in order of use from left to right on the righthand side of the plate. That is, if your first course is salad, put the salad fork to the far left so that it is closest to the person's plate and easiest to reach with the right hand. Then the larger dinner fork would go to the right of that, etc.  But I'm not sure about the really fancy etiquette rules - I'm sure it'd be in Emily Post or Miss Manners or one of those etiquette guides.
