  1. Sunshine,

The "unclaimed dividend" story is a scam.  It is just like the spam e-mails you get about someone rich having died in another country and someone needing your help (for which they will pay you) to collect some of his money.  Please do yourself a favor and ignore anyone who tells you otherwise.

  2. In the US there is not standard firm used by broker/dealers to be used by customers for claiming dividends.

If the firm is holding the dividends, your broker should be able to provide the information you need. BUt...

The dividend should have been posted to your account, if not call the customer service section of the firm and discuss it with them.  Dividends are posted systemically, there may be the possiblity you were not entitled to the dividend.  If customer services wants something in writing, send them what they ask for AND send a copy of that letter to the Compliance Department.

If the firm already escheated the dividend you the state, you have to contact the state where you live  (can be done on -line) and they will provide you with the paper work you need, not all states use the same form.
