I say commando but I see a few old guys wearing speedo's in the parking lot.  They don't have to cover up to change in/out of their wetsuit.  But for the other 95% of those who surf, you wear nothing.

Any item of clothing worn under a wetsuit is just dead space between your body (the source of heat) and the wetsuit (the thing that retains that heat).  This dead space fills with cold water.  Because of this, if you wear anything (and I don't) make sure it is tight-fitting and thin material.  Women often wear bikinis and some men wear speedos, just to make it easier to change.  I doubt these have any affect on temperature, but I would certainly never wear boardshorts, or boxershorts, or else that area of my body would be quite cold...

Nothing is needed to be worn under a wetsuit. I have done the following from time to time. If I was going surfing somewhere that I had to walk down the beach a ways, I would were boardshorts underneath so I didn't have to wear my wetsuit the whole time. They are a pain when you first put the wetsuit on though, because they ride up. I see alot of other guys wear a rash-guard underneath, but I never saw a point for it.

Alot of guys will buy a jammer or racer, and the girls usually wear a bikini underneath their wetsuits.

I'll wear speedos to make it easier to change in and out of the wetsuit without worrying about covering up with a towel.  If I'm wearing my 4/3, I don't wear anything else under.  If I have my 3/2 and it's a little chilly out, I'll either wear a rashguard or a .5mm neoprene shirt to add a little extra warmth.
