I assume you're referring to euthanasia?  Is "Utopian" a specific philosophical perspective, or are you just assuming that everyone who believes in utopia would treat incurably ill people the same way?  Because there are many different perspectives on what a true utopia would be, and whether it is even possible.  Anyway, as to your question: even if someone is in pain, they could still enjoy life, and if they do still enjoy life, their life is still worth something.
