  1. In my experience, all four options are correct.

    All your clients will want to know how their testimonial is going to be used and how their name will be associated with your product or service. So you will want to have this clear in your mind before you ask for one.

    Also, if you are doing print ads, tv spots or radio commercials, it might be a good idea to have a "sample script," so they can see how their testimonial might be used.

    When you ask for testimonials, you might want to provide your clients a list of "talking points." This will help them focus on the key messages you wish to cover. For example, if you are particularly proud of your customer service, you might ask your clients to speak to just that point.

    You will also find that some clients are just not comfortable writing their own testimonials. So for them, you might want to offer to interview them and write up a testimonial based on their comments.

    Whenever you solicit and/or accept a testimonial, you will want to provide the client with a written statement that explains how the testimonial is to be used and who retains ownership of the testimonial and/or final ad. This document should also detail any compensation agreement. This does not have to be a lengthy agreement.

    I've attached a link to a site that has a sample Talent Release Form. You will want to revise the document to read Customer Testimonial Form.

    This document will prevent your client from coming back to you later and asking for more money and/or issuing a cease and desist order for the unauthorized use of their name or likeness in your promotional materials.

    so submit a survey to them with choices and piece togeather anarration from there
