  1. That would depend on how you personally define "safety"

    In terms of crash ratings, there hasn't been a US airliner crash since the Comair crash in Lexington, and before that is was 2001.  So I guess you could say that since then, all airlines except Comair have a perfect safety record.

    However there are occasional mishaps that don't involve crashes (AirTran had a fan blade shred an engine and the shrapnel killed a passenger in 1999 I believe) but those are so rare and random that they are difficult to quantify.

    A company like American Airlines or Southwest Airlines carries so many passengers that a certain percentage of them die of natural causes aboard their aircraft every year.  Heart attacks, strokes, and such happen.  Not often, but they happen.

    In terms of the airline that has the lowest incidence of FAA violations, I believe that is Southwest, but for many years SWA enjoyed a very cozy relationship with the FAA where they got away with things that most airlines were not allowed to do.

    I guess what i'm saying is that there's no ONE answer to your question.  There are many and the answer is complicated.  If there was one airline that was ranked number one, you can probably bet that would be in every single one of their commercials and on every billboard in the US.
