  1. one major constitutes a fail

  2. I'll try to interpret your question.  You are asking what are major violations that may  cause you to fail a driver's test, right?

It only takes ONE bad one, like failing to yield right-of-way, driving too fast, or driving through a stop light or stop sign, for example.

Other minor violations (which at discretion of tester) could cause you to fail too.  Like, inability to start your car, not knowing how to turn on head lights, or safety warning blinkers, for example.

You may be asked  to open hood of car too.

And you could fail if you cannot shift smoothly, if you drive a standard transmission.

Drive in acordance with posted speed limits, and use your turn signals within proper distance of required turns. All these "tests" will be considered, and failing one or more could be fatal, all at discretion of tester.

Hope this helps you "pass", and not fail.
