What do I need to change my name? I want to keep my name but add a middle name.

Currently I have only two names, but I would like to have three like a normal person. My brother and I have the same initials and I want mine to be different. What would I need to do the proper name change? Will I need my social security card and my birth certificate?

  1. Yes, you will need your SS card, your birth certificate, and photo ID.  You may be required to have other information, too, depending on your state or county of residence.  If you're under 18, you'll need a parent with you.

    You do name changes at a courthouse.
