What do I need to do to file a restraining order?

What kind of proof do I need?

I feel that I am in danger because I can't sleep, I'm constantly looking over my shoulder, when she comes around me I get nervous that I'll be injured, I'm afraid she'll wait outside my work and beat me up. She has never threatened this but I feel danger.

A police report then you go before a judge who examines the evidence and decides  if an order of protection is warranted.
