What do I send along with the Articles of Incorporation in California?

I am filing paperwork to create a nonprofit symphony orchestra - I already have my Articles of Incorporation written and a check made out, but what does the Secretary of State mean when she asks for "a letter referencing the corporate name as well as your own name, return address and telephone number?"
